PresentationThe Bloom Program in Nova Scotia - a prototype pharmacy service

When brainstorming the future of pharmacy, discussants describe working at one's full professional scope, investing in patient-centred, longitudinal clinical services, and an enabling payment model that is independent of product or medication sales. The Bloom Program, which is a new mental health and addictions community pharmacy program, aligns with these ideas. The program emphasizes collaborative, longitudinal, holistic, patient-centred care. To offer the program, pharmacies in Nova Scotia must be approved by the provincial program coordinator after completing a comprehensive 9-step application. They then enrol eligible patients into the program, providing them with enhanced care and support over 6 months to address their prioritized health and medication problems. The origins of the program, its current status, and lessons learned will be discussed at this session.
