As the Pharmacist in this role you will work with the inter-professional team to provide clinical pharmacy services to a wide range of patients which includes providing a range of drug distribution and clinical pharmacy services. These services will include health promotion, disease prevention, chronic disease management as per the college’s scope of practice. In addition you will participate in patient/provider education and provide drug information to nursing and clinical staff. This role will work closely with clinicians, an inter-disciplinary team, and pharmacy technicians.
- Licensed (Part A) with Ontario College of Pharmacists
- Current injection certification preferred
- Membership with the Canadian Society of Hospital Pharmacists (CSHP) (preferred)
