Developing a Practice Guidance Tool for Prescribing Cascades in Primary Care Teams
Dr. Toenjes will share findings from qualitative research with Ontario’s primary care teams that sought to enhance understanding of strategies for addressing prescribing cascades. Prescribing cascades are an important and under-recognized contributor to medication-related harm that occur when someone takes a medication and experiences a side effect which is then treated with another medication.
Speaker: Sameera Toenjes
Dr. Sameera Toenjes is a pharmacist who practices as part of the interprofessional team at Women’s College Family Practice Health Center in Toronto, Ontario. She completed her BSc in Pharmacology from McGill University, her PharmD from the University of Toronto, and her pharmacy residency at Women’s College Hospital. She recently completed her MSc studies at the University of Toronto.
Discussant: Barb Farrell
- Dr. Barbara Farrell is a pharmacist with 25 years of experience working in the Bruyere Continuing Care Geriatric Day Hospital and a scientist with the Bruyere Research Institute. She is the Founder and Senior Advisor with Deprescribing.org, the research team that develops and studies implementation of deprescribing guidelines. She has also conducted qualitative studies examining healthcare provider roles in managing polypharmacy as well as healthcare provider and public perspectives about prescribing cascades.
