Dear faculty, staff & students,
 
As a follow up to the message below, please be advised that we are updating the sign-in process when entering the Leslie Dan Faculty of Pharmacy Building.
 
As of Wednesday, December 9th, all building visitors must scan a QR (Quick Response) code (or fill out a paper form if they do not have a cell phone), and complete an online contact tracing form. Reducing paper forms minimizes touch points, speeds up the sign-in process, and facilitates contact tracing.
 
Most mobile phones recognize QR codes.
 
Instructions:
 
  1. Open your camera app
  2. Point it at the QR code posted at the sign-in table
  3. Follow the website URL that pops up.
 If you have an older phone, you can use any QR code scanning app or website that recognizes QR codes.
 
We encourage you to use the QR code, but if you do not have a phone at the time of entering the building, a paper form will still be available at the sign-in table. Please ensure you bring your own pen.
